Job Title: Intake/Placement Coordinator
Job Summary: The Intake/Placement Coordinator is responsible for managing the intake and placement process for new residents entering the treatment center. This role involves assessing incoming clients, gathering necessary documentation, coordinating placement options, and ensuring that each client is placed in a program that best matches their treatment needs. The Intake/Placement Coordinator also serves as a liaison between the client, their families, healthcare providers, and the treatment team to ensure a smooth and supportive admission experience.
Key Responsibilities:
1. Client Intake & Assessment:
- Conduct initial assessments to determine the client’s treatment needs, including gathering personal, medical, psychological, and social history.
- Ensure all necessary documentation is obtained, such as insurance information, consent forms, and medical records.
- Provide a comprehensive overview of the treatment center’s services, policies, and procedures to new clients and their families.
- Work collaboratively with the clinical team to assess whether the center's programs align with the client’s needs.
2. Placement Coordination:
- Review client assessments to determine the most appropriate treatment program based on the client’s clinical and personal needs.
- Coordinate with various departments within the center to ensure smooth placement (e.g., medical, clinical, behavioral health, and housing).
- Work with families and external referral sources (e.g., hospitals, therapists, social workers) to ensure timely and appropriate placement.
- Assist with the scheduling of admissions and prepare clients for their entry into the program, including orienting them to the center’s rules and routines.
3. Communication & Collaboration:
- Act as the primary point of contact for families, clients, and external agencies during the intake and placement process.
- Provide clients and families with necessary information regarding treatment options, program expectations, and any required documentation.
- Facilitate communication between the client and other team members (e.g., case managers, therapists, medical staff) to ensure all relevant information is shared for optimal care.
- Follow up with clients and families post-placement to assess satisfaction and address any immediate concerns.
4. Documentation & Compliance:
- Ensure all intake documents are accurately completed, maintained, and filed in accordance with the center's policies and regulatory requirements (e.g., HIPAA compliance).
- Document client progress, concerns, and outcomes throughout the intake and placement process.
- Assist with billing and insurance verification as needed, ensuring all financial documentation is complete and accurate.
5. Crisis Management:
- Assess and manage urgent or emergency placement needs when necessary, coordinating immediate admissions if required.
- Work with the clinical team to determine any special accommodations or safety considerations for high-risk clients.
Aftercare Coordinator:
- Primary Role: The Aftercare Coordinator focuses on supporting clients after they have completed an initial treatment program or service. Their goal is to ensure clients maintain progress and continue to receive the necessary support for long-term success.
- Key Responsibilities:
- Developing and monitoring aftercare plans for clients who have completed treatment or a program.
- Coordinating follow-up services, such as therapy sessions, support groups, or additional treatments.
- Providing ongoing support and resources to ensure clients’ continued success.
- Communicating with clients to track their progress and address any concerns.
- Assisting clients with any challenges they face post-treatment, such as adjusting to new environments or managing personal issues.
- Ensuring that clients have access to necessary community resources.
- Keeping accurate records of aftercare services provided and monitoring clients' progress.
Both roles are integral to ensuring that clients receive comprehensive care throughout their journey. Qualifications:
Education:
- Bachelor’s degree in Social Work, Psychology, Nursing, Healthcare Administration, or related field (preferred).
- Relevant certifications in mental health or substance use treatment (if applicable).
Job Type: Full-time
Pay: $35,000.00 - $40,000.00 per year
Schedule:
- Monday to Friday
- Weekends as needed
